Our Mission
Established in 2019 at the University of Michigan School of Public Health, the Social Care in Chronic Disease Collaborative advances the science on addressing social context in chronic disease management.
The Challenge:
Social Factors are a Major Barrier to Managing Health
Despite significant medical advancements, chronic diseases continue to be a leading cause of death and disability. A critical, often overlooked factor is the social context in which people live. Issues such as food insecurity, housing instability, transportation barriers, and financial burdens prevent millions from effectively managing chronic conditions like diabetes. This creates profound health disparities and undermines our ability to achieve true health equity.
The Social Care in Chronic Disease (SCCD) Collaborative is a multidisciplinary team dedicated to solving this problem. We believe that by systematically integrating social care into chronic disease management, we can dramatically improve health outcomes. Our mission is to advance the science of this work through rigorous research, evaluation, and innovative translation of our findings into real-world practice and policy.
